Geriatric Care Assessment can provide practical solutions to local and long distance caregiving challenges by joining family caregivers with a geriatric care manager in their older relative’s home area. Unlike costly face-to-face geriatric care management services, Care Coach allows for families to hold a telephonic “family meeting” with a qualified geriatric care manager.
